Summary

For‑profit entities and individuals are not eligible to apply, and the focus is on organizations with experience or willingness to engage in policy, systems change, community voice efforts, or research. Funds are typically used for activities such as expanding access to services through innovative care models, strengthening advocacy for equitable policies, engaging underserved populations in developing solutions, and conducting research that informs improved practices and policy.


The funding interests include policy and advocacy work, community capacity building, improving care delivery models, and research that generates new insights into health disparities and solutions. Projects can range from initiatives that engage community members in shaping solutions to those that test or refine models of care delivery, including integrated or minimally invasive approaches. Research projects that explore factors influencing inequities and evaluate intervention outcomes are also supported.


There is flexibility in award amounts based on the scope and strategic fit of the proposed work, rather than set minimums or maximums, and proposals are evaluated on how well they align with priorities aimed at reducing inequities and fostering systemic improvements. The application process generally involves submitting a concept statement, with multiple submission cycles throughout the year, followed by full proposals if invited. Grants are awarded through recurring cycles and can support work on annual or project‑specific timelines.
